Provides a variety of services on health issues, regardless of income, including immunizations, prenatal care coordination, head lice detection and education and confidential HIV testing. Houses all birth and death certificates for those events that occurred in Boone County. Provides flu shots at a reduced rate in October and November. The Environmental Division inspects and issues permits to
all food establishments and vendors and responds to
complaints regarding food handling and quality. Records
all animal bites and facilitates rabies testing and water
sampling. Responds to all complaints indicating potential
impact on public health or safety.

Provides all types of medical assistance such as ambulance,
emergency room, inpatient, radiology, laboratory, etc. If
you cannot afford services, assistance can be provided at no

Provides healthcare for the non-insured and under insured
in Boone County. Fee is based on income and family size
with minimal charge. Services offered are: Women's
Health, Well Child check up, routine health care, and

care, healthy beginnings, children with special health care

Women, Infants & Children Program (WIC)

Provides a special supplemental food program for women, infants, and children. Available to all eligible participants including pregnant women, breast-feeding mothers, postpartum non-breast-feeding mothers, infants and children up to five years of age. Participants receive vouchers redeemable for specific nutritious foods at designated grocery stores, nutrition education, nutrition counseling, and referrals to other health and social services. Conditions include living in Boone County, have a nutritional need and meet income guidelines. All persons who receive Medicaid, Food Stamps, or AFDC are incomeeligible for WIC. Now offering free pregnancy testing during clinic hours.

Provides for adults not old enough for Medicare and many
seniors who do not have supplemental health insurance
for health care; and medications and financially unable to
obtain primary medical care. Promotes preventive health
checks, such as vision, blood pressure and cholesterol
screening; urine analysis; prescription counseling; and flu
shots.

Provides special transportation, wig cupboard and
assistance with purchasing mastectomy breast forms
and bras for mastectomy patients. Also offers medical
screening, stop-smoking information, anti-smoking
education program for schools, public education and
equipment loan.

Offers limited financial aid for medication and other
necessities for low income mental health patients and
battered women and their children. Provides programs on
self-esteem, job search skills, nurturing parents at no cost.

Administers TANF, Food Stamps and Hoosier Healthwise/
Medicaid programs inn Boone County. Provides hospital
care for the low income in Boone County. Also provides
care coordination and financial assistance for primary
and specialty program medical services to Boone County
children birth to 21 who are financially and medically in
need of health services as the result of chronic illness or
disability. Families must meet medical and financial criteria
and apply for Medicaid. Call for more information.

Provides nutrition counseling, supplemental food and health referrals to pregnant or breastfeeding women and children birth - 5 years.
INDIANA 2-1-1
Basic needs such as food, housing, utility assistance and
transportation. IN211 Centers maintain databases with
thousands of resources to meet a variety of needs. Most
wireless phone customers do not yet have access to 2-1-1,
but IN211 hopes to offer wireless access in 2005.
WHAT IS INDIANA 211- A nonprofit organization that
works with many partners and relies on financial support
from Cinergy Foundation, United Ways, Indiana State
Department of Health, Indiana Family and Social Services
Administration, other funders and individual donors.

Energy Assistance Program
(Community Action Program)

Assists low-income families with the high costs of home
energy. Applicants must be within program income
guidelines. Energy costs covered include electric, gas, oil,
LP, gas, etc. Assistance with restoring disconnected utilities
or preventing disconnection. Households with heating
emergency will be given priority.
Housing Assistance

Provide monthly housing assistance payments. The tenant
pays a portion of the rent based on their income. The
assistance is ongoing as long as the tenant remains eligible
and complies with program regulations. Applicants must be
within program income guidelines.
The Caring Center

Located on the ground level of Central Christian Church.
Open weekdays, or by appointment in an emergency.
The Center also distributes Thanksgiving and Christmas

Provides a structured, managed home environment in
which clients in life transitions can have a chance to rest,
gain strength and stability, find stable employment and
make positive changes in their lives so they can move
toward independent living. Admission to the program is by
application on a space available basis. The apartment house
also provides short term, emergency shelter for transients
and homeless persons or abuse victims.

Offers a monthly, affordable, supplemental and nutritious
food package (worth $28-$30) to participants for a
small fixed fee ($15 cash or food stamps) and 2 hours of
volunteer service to the community. Participation is open
to everyone; no income or other eligibility guidelines.
Orders are taken at the beginning of each month and food
is distributed at the end of each month. Call for sign-up and
distribution times.
